Wording every single-live-instance refusal contains (lower-cased), so the failure classifier and callers can recognise one without a type.
Only refusals that mean another instance holds the agent. acquire’s
lease-file I/O failure (“… Refusing to start it twice.”) is deliberately
not one: it is a local, possibly transient error, and classifying it as
“running in another instance” would offer Take over instead of Retry
(ReAgent P1 on #3742).
