A scanned directory is repository-controlled and unbounded — nothing stops a repo holding thousands of *.instructions.md files. The declared paths above are a fixed handful, so only this side needs a limit. On overflow the scan reports what it found plus an explicit marker entry, rather than silently returning a prefix: a truncated list that looks complete is the failure mode this module exists to remove.
