Live status of a running (or recently exited) shell. Updated by
ShellNodeRunner as output arrives and on exit.
block_id/cmd/title/started_at are populated once at spawn (see
ShellNodeRunner::run) and never change — kept here (rather than a
separate lookup) so list_active (SPEC_SWARM_LONG_RUNNING_PROCESS_
ROWS_2026_07_20) can answer “which shells belong to block X” from the
same map get_status/ShellStatus already query, with no new registry
or second source of truth.
