Seed knownEntries from the init RPC snapshot. Called once from
app-init.ts::initInstanceTracking after listWindowInstances
returns. The reducer’s ApplySeed arm preserves existing entries
(codex P1 #603) and skips tombstoned labels (codex P2 #603).
